1. Bank Statement (or Bank Account Details)

Purpose:

  • Used to set up payroll and ensure salary is deposited into the correct employee account.

  • Serves as financial identity verification when the employee does not have a payslip history.

Helps confirm the employee’s name matches their registered bank details.

2. SSS ID Number (Social Security System)

Purpose:

  • Mandatory for enrollment in the government’s social insurance program.

  • Ensures Globalli can remit contributions for sickness, maternity, disability, retirement, and other member benefits.

  • Used for verification during SSS reporting and claims.

3. Tax Identification Number (TIN)

Purpose:

  • Required for correct tax withholding and reporting to the Bureau of Internal Revenue (BIR).

  • Ensures the employee is properly registered for income tax compliance.

  • Needed to issue BIR Form 2316 annually.

4. PhilHealth ID Number

Purpose:

  • Used to enroll and remit contributions to the national health insurance program.

  • Ensures employees can access PhilHealth benefits for medical, hospitalization, and outpatient services.

  • Needed for claims processing and verification.

5. Pag-IBIG ID Number

Purpose:

  • Required for the Home Development Mutual Fund (HDMF) membership.

  • Ensures proper remittance of Pag-IBIG contributions.

  • Allows employees to access Pag-IBIG loans, savings programs (e.g., MP2), and housing benefits.

6. Proof of Address (e.g., Utility Bill, Barangay Certificate, Bank Statement)

Purpose:

  • Used for identity and residency verification.

  • Required by some banks, government agencies, and for payroll compliance.

  • Helps validate employee location for tax and statutory reporting.

7. BIR Form 2316 / Waiver of BIR 2316 / Year-to-Date (YTD) Payslips

Purpose:

  • BIR 2316: Shows the employee’s previous employer’s income and tax withheld for the year.

  • Waiver of 2316: Used when the employee cannot provide the form due to employer circumstances.

  • YTD Payslips: Alternative proof of year-to-date taxable income and withholdings.

These documents ensure:

  • Accurate tax annualization.

  • Correct withholding tax computation for the remainder of the year.

  • Avoidance of overpayment or underpayment of taxes.

8. Visa / Work Permit (for Foreign Employees)

Purpose:

  • Required to legally work and reside in the Philippines.

  • Ensures Globalli compliance with immigration laws.

  • Common permits include the 9(g) Work Visa, PEZA Visa, Special Work Permit (SWP), or Provisional Work Permit (PWP).
